Introduction
Bromocriptine Mesylate is an ergot derivative which acts like a natural substance found in the brain (dopamine). It helps to reduce the levels of certain hormones found in the blood like prolactin and growth hormones. It helps to prevent diseases caused due to the excess of these hormones, however, it is not a cure for them.
Use
Hyperprolactinemia - Bromocriptine Mesylate tablets are indicated for the treatment of conditions associated with hyperprolactinemia, including amenorrhea with or without galactorrhea, decreased sperm production, or infertility. It can be used before an adenectomy (surgical removal of endocrine gland) to reduce the size of tumor before surgery.
Acromegaly - It is indicated in the treatment of acromegaly since it reduces the levels of growth hormone significantly.
Parkinson's Disease - It is also indicated in the treatment of Parkinson’s disease, in conditions where there is the development of tolerance to carbidopa or levodopa and to reduce the side effects (nausea, hallucination, and confusion) associated with extended dosage.
This drug is known to stimulate post synaptic dopamine receptors, which regulate the amount of secretion of prolactin.
Dosage and administration
This drug is available in tablet form in the strengths of 1.25 mg, 2.5 mg, and 5 mg. It is to be taken orally with food.
Hyperprolactinemia and Acromegaly - The initial recommended dose is for the 0.5 to 2.5 mg tablets that are to be taken daily. Additonally, the 2.5 mg tablets may be added to the regimen depending on the patient’s tolerance every 2 to 7 days, until the desired response is achieved.
The daily drug dose should range between 2.5 to 15 mg for hyperprolactinemia and 20 to 30 mg for acromegaly.
Parkinson's Disease - An initial dose of the 0.5 to 2.5 mg tablets, to be taken twice daily. This dose should be gradually increased (2.5 mg every 2 to 4 weeks) until the desired response is achieved. The dose of levodopa should be maintained during this period.
The dose is gradually tapered to reach a maintenance dose at which the desired effect is achieved.
